Give Solo Practitioners Treatment Plan Memory That Holds Across Every Patient Visit
Solo practitioners — therapists, physical therapists, chiropractors, acupuncturists — carry every patient's treatment plan in their head between visits. AI tools see one visit at a time. MemoryLake gives solo practitioners persistent treatment plan memory across visits.

The problem: solo practitioner AI restarts every visit
You've been treating this patient for six weeks. The plan has evolved through three iterations based on response. Today's AI scribe sees only this visit's notes. The trajectory of the treatment plan stays in your head, where it's hard to share with covering practitioners.
How MemoryLake supports solo practitioner treatment plan memory

How it works for solo practitioner treatment plan memory
- Connect — Authorize EHR or practice management tools.
- Structure — Treatment plans and responses become typed memory.
- Reuse — Every visit's AI tools retrieve full plan history.
Before vs. after: solo practitioner treatment plan memory
| Without MemoryLake | With MemoryLake | |
|---|---|---|
| Plan evolution visibility | Lives in head | Captured |
| Covering practitioner handoff | Painful | Memory transfers |
| Cross-visit response patterns | Manual recall | Reflection memory |
| Audit plan rationale | Limited | Memory provenance |
Who this is for
Solo and small practice practitioners — therapists, PTs, chiropractors, naturopaths — where treatment continuity is professional standard but technology doesn't currently support it.
Try this use case on your own account
A solo physiotherapist's plan for one patient has been rewritten twice, each time by an AI scribe working from the previous version and that day's visit. Between them, two of the three original precautions fell out: no overhead loading until the orthopaedic review, and no dry needling because he takes an anticoagulant. Nobody lifted either one, and the current plan now schedules both. The covering clinician is told to follow the current plan.
What you will see
- Every chart entry is one fact on the patient's actor, word for word, with
fact add;fact update --metadatarecords what it is: a plan version and the precautions it carries, a precaution being imposed, revised, reaffirmed or lifted, or a visit note - The quick read: take the newest plan (
fact list --actors <patient>). It lists one precaution, and all 5 items of the covering clinician's first session come straight from it, overhead press and dry needling included - The audit is plain code over the same list: each precaution from the day it was imposed, through every plan version since. Two were dropped in the scribe's v2 and never lifted (both were repeated at later visits); the pain ceiling changed on the record and v3 carries it. 2 of 5 session items go on hold. Run as of v1's date it flags nothing (3 of 3 carried)
- A keyword check passes:
fact list --query overheadmatches v3 itself ("add overhead dumbbell press"). The handoff is written back withmetadata.basislisting every chart entry it rests on (7 of 7 read back). Plus a local web UI
